Warrior BV uses the details in this form to review your sign-on, contact you about your berth and prepare for the voyage. The skipper may need to share required crew details with port and customs authorities. Your details are not a public crew list. You may ask to see, correct or remove your details by writing to willem@sailwarrior.com. The privacy notice explains this in more detail.

Health information *

Offshore, medical help can be days away. I agree that Warrior BV may collect and use the health information I give (allergies, medication, relevant conditions) to judge whether I am fit to sail, plan safety on board and respond to an emergency. Emergency-relevant details may be printed and kept on board, available only to the skipper and watch leaders. I can withdraw this consent by email at any time, understanding that we may then be unable to take me on the voyage.

This consent covers health information you choose to provide when preparing for a voyage. It does not ask you to enter medical details in this sign-on form. Relevant allergies, medication and conditions help the skipper assess fitness to sail and prepare for an emergency. Only the skipper and watch leaders may see emergency-relevant details kept on board. To withdraw consent, write to willem@sailwarrior.com. If we cannot prepare safely without those details, we may be unable to take you sailing.

Conduct and risk *

I will treat everyone with respect. Harassment is not tolerated. I will follow the skipper's safety instructions and will not be impaired aboard. Offshore sailing carries real risks, including heavy weather, injury and delayed medical help. I take part voluntarily and accept these risks. A safe crew looks after one another. I will respect other people, their boundaries and the shared space aboard. I understand that there is zero tolerance for harassment. I will follow the skipper's safety instructions and will not let alcohol, drugs or any other impairment affect me aboard. Heavy weather, equipment failure and injury can occur, and medical care may be days away. I choose to participate with an understanding of those risks. I will provide truthful information and report changes relevant to my safety or the voyage. This statement does not remove any rights I have under applicable law.
